In this workshop participants will gain knowledge and insight into the PfA pathways when a young person is preparing for higher/further education and/or employment. Participants will explore how the Education, Health & Care (EHC) Plan supports young people with SEND to achieve independent living, maintain good health become a valued part of their community as a young adult, and build and maintain friendships. The workshop supports participants to understand what is available in Hertfordshire to prepare young people with send for adulthood, and gain insight from other professionals on the different services to understand how they work with, and support young people to prepare for adulthood. Participants will understand the importance of PfA within our EHC Plans and how this is underpinned by the SEND Code of Practice and key legislation.
Course Objectives:
- At the end of this session participants will be able to:
- State the key PfA pathways
- Explain how PfA pathways can support children and young people from the earliest age
- Signpost families to information on PfA and services available
- Convert areas of need into PfA pathways
- State what is needed to ensure EHC Plans contain high quality PfA
